Fight
Against External MD

An appeal by the All India Association of Post-Graduate Homoeopaths

This is the copy of a memorandum submitted by AIAPGH to various authorities concerning the External MD course in Homoeopathy. We reproduce it in full due to the importance of the problem and urge all self-respecting homoeopath to take a copy of this and send to the authorities mentioned in the last part of this memo.

 Dear Sir,

Sub : Considering the MD (Hom) in academic field for the cancellation of proposed external MD course in Homoeopathy.

By the introduction of the regular Post-Graduate studies in Homoeopathy, our system took a giant leap towards improving the standard and the quality in par with other prevalent systems of medicine. But by trying to propagate and introduce the External M.D. programme as explained later, our Council has degraded the sole purpose of the Post-Graduate studies. It is very unfortunate to mention that the regular M.D. holders were not at all considered as teachers, examiners and guides for the present regular Post-Graduate students; but the Diploma holders were encouraged and considered for the same. This fact may be strange, but is sadly true. At present active movements are taking place in Kerala also to start External MD.

We would like to bring to your notice that in the past years, Post- Graduates in Homoeopathy were quite few in number, as there were only few colleges offering MD. Due to the unavailability of sufficient M.D. hands in Homoeopathy, the Diploma holders who had an experience of twenty years, which included ten years of teaching in the particular Departments (Materia Medica, Organon & Homoeopathic Philosophy and Repertory) were appointed as guides, examiners and teachers to the Post-Graduate students.
Before any M.D. regular courses were started in all systems, there was a period when the external M.D. courses were propagated to improve the standard and quality of the system. But as soon as a proper curriculum for a regular MD was formulated the former was immediately stopped and taken over by the latter to take the system to its next higher level of existence. Various additions to the syllabus, more intense clinical and bed-side training and much more helped in improving the quality and standard of M.D. programme; which is absurdly totally lacking in the external M.D. programme.
It is nothing less than a crime if we, in this era of advancement of science and medicine, start to use those outdated and substandard tools such as external M.D, ignoring and disregarding the more efficient regular M.D. course to propagate our great system. It becomes a mockery, not just to anybody who believes in the advancement and progress, but to the very system of Homoeopathy.
We appreciate the suggestions made by Central Council of Homoeopathy that all in the teaching faculty should be a M.D. But the value of the M.D. should not be degraded by conducting such courses as an external M.D. programme, which is equivalent to any correspondence course. As study of medicine is directly related to public health, a mere correspondence course (external M.D.), the eligibility for which is his/her experience in the field; will question the very basic concept of 'Law", that it should be of 'equity and good conscious'. If we look into this very conflict of external M.D., it is crystal clear that it is just an outcome of the insecurity feeling of various individuals who want to gain the benefits and privileges of the prestigious designation M.D. through much easier and stress free processes. If what we want is only an MD to all the teaching staff why not confer them all an MD without even conducting a course? Similar is the foolishness implied in External MD.
Earlier when we had approached the concerned Universities regarding the same, we had received more than a positive response and they welcomed such innovative and spirited ideology and communicated to the Central Council of Homoeopathy regarding the same. But the C.C.H. on the other hand outrightly ignored this plea by not responding. This particular stand of C.C.H. shows that they are retarding the growth of this true system of medicine. By doing so, the sitting councilors deliberately misuse the power of an association conferred to them.
Currently we have got sufficient M.D. Graduates, who are ready and willing to impart their valuable services to the new generations of M.D. Homoeopathy, but it is very disappointing to note that our Council, Universities and Government have ignored this potential. The various training programs for the Post-Graduates are still handled by Diploma holders and not a single M.D. (Regular) holder have been given the opportunity to become guide, teacher and examiner at present.
The transitory provision for enabling the Diploma holders to take-up M.D. course for a period of 8 years from 1989, has no justification to be continued as a regular feature for another 8 years when hundreds of P.G. holders all over the country have been produced by various P.G. centres of studies in Homoeopathy. “It is imperative the continuation of the external Degree is only for the purpose of sub- serving the vested interests of a group of teachers with Diploma/Degree in Homoeopathy.
The above said 8 years' period came to an end around 1997. In between, in the State of Karnataka alone more than 100 P.G s’ have come out successfully. There were five batches from Belgaum one from Gulberga and two from Mangalore. From the state of Kerala also more than 120 Post-Graduates (6 batches) have come out. At present in Karnataka there are about 4 Colleges imparting P.G. courses in Homoeopathy. Thus there is no dearth of Post-Graduate candidates. Then, there are all over Karnataka 13 Colleges imparting under graduation in Homoeopathy to around 750 students, out of which at least 500 students will be qualifying every year. In Kerala 250 Graduates are passing out every year. Therefore, there is no dearth of graduate hands also, who is the real inheritors of P.G., for taking-up P.G. studies in Homoeopathy. Also there are enough feeding for P.G. Colleges in Kerala, which have an intake of 24 students a year in all. This is the situation in detail regarding South India. Same is the situation on an all India front also. Maharashtra alone accounts for more than 40 Colleges with an intake of 40 P.G. students per year.
Therefore, the need, if any, for allowing Diploma holders to take-up Masters' Degree in Homoeopathy in the course of 8 years has been completely fulfilled. Therefore, the legislature has rightly put an end to it. In between Graduate / Diploma holders engaged in teaching job are also permitted to take-up regular course of two years' Post-Graduation Degree as a special case. Thus the amendment made in 2001 rightly deleted the Diploma holders from the eligibility clause for regular M.D. course. Only Degree holders were allowed to stake their claim to P.G. course as per the amendments under Regulation 4.
The following facts make it very clear that the distinction between the P.G. course offered to external candidates and to the regular candidates is very fundamental and the former is grossly inadequate and destructive to the system.
The regular candidates have to study for 3 years whereas the external candidates study the P.G. course only for 2 years. The external candidates register their names and carry on their studies through correspondence only whereas the regular students attend the classes every day. They will have out-patients and in-patients to attend to and teaching of under graduates is also compulsory in the curriculum. Besides, there would be regular seminars, presentation of papers and clinical discussions etc. for regular candidates. There is also a Journal Club for the regular candidates. In addition, the regular candidates have definite marks to score in each paper in practical as well as viva voce and case presentation whereas this feature is conspicuously absent from the examination schedule of the external candidates. The external candidates have to offer only two special subjects, one medicine and one subsidiary, in all 4 papers of 100 marks each at the end of the second year. They would not get any clinical training, teaching training during the period of study whereas the regular candidates will have to appear at the end of the second year for papers carrying 800 marks including viva and practicals. At the end of 3rd year they have to appear for another 750 marks worth papers. The thesis presentation is in addition to all this.
Thus the gap between regular candidates and the external candidates is very wide. The continuation of the external P.G. course would defeat the concept of educational standards and excellence in the system. As a result, the patients would be precariously exposed to danger at the hands of Post- Graduate Homoeopaths having received the training as external candidates. Yet, surprisingly the amendment to the regulation in 2001 has continued the system of permitting external Post-Graduation for another 8 years for the benefit of the teachers, who hold Diploma/Degrees. Here it may be worth pointing out that at the helm of affairs, policy making for prescribing standards of education, research etc. are manned and controlled by those with only diploma. They obviously should be keen on sub-serving the interest of their colleagues.
Thus the continuation of external course in the Post-Graduate education has been done only for sub-serving the cause of a limited class of teachers. To masquerade this a provision is there for diploma holders with 10 years professional experience also to take-up external P.G. course.  Which is even more dangerous in that anybody who have 10 years in record after his/her passing out with a Homoeopathic diploma can get a P.G. irrespective of how they acquire the same. We should also consider along with this the standard of the diploma courses conducted by many institutions as well as the eligibility criterion for the same.
It is clear from all these that the system of external P.G. Degree is irrational, arbitrary and would endanger the system.   The system of conferring Post-Graduate Medical Degrees by correspondence is opposed to all notions of organized training/education because such a degree with a shorter course and with no clinical and teaching training is adverse to the interest of the excellence and standard of education. It will only degrade and disintegrate the system of Homoeopathy in the long run. Not only that but it would endanger by exposing the lives of the people to the hands of ill-trained doctors.
The discrimination is written in bold letters as the benefit of this special provision to do external P.G. Degree-is not available to Degree holders with required teaching experience and it is limited only to diploma holders. We request that the Central Council of Homoeopathy should not encourage the external M.D. courses to maintain the standard of Homoeopathy in our country as it is clearly degrading it.
The Association has filed a civil writ petition at Delhi High Court on 18.12.2001, No.7716/2001 challenging the validity of the notification issued by CCH dated 31.10.2001  as being arbitrary, irrational, discrimination and violation of fundamental rights under article 14 of the constitution of India.

Basic qualification for a Post- Graduation is at the minimum a Graduation in all the universities. But External MD is a PG without a Graduation!
We hope that the above matter will be dealt with grave seriousness and necessary steps would be taken to resolve the situation. Expecting a favorable and positive response from all graduates in Homoeopathy.

Thanking you,
Yours faithfully,
Dr. K.R.Mansoor Ali,

An appeal to all the medical students
(Taken From Homoeopathic Medical Panorama Vol: 10 No: 3)
One more batch of students is entering the various colleges in Kerala including the Homoeopathic Medical Colleges. This is an appeal to all the medical students who are the aspiring young physicians of tomorrow. This is equally applicable to all the senior students as well the practitioners in this noble profession.

Dear aspiring young physicians. Please understand that the future of the system is in your hands. You are the successful practitioners of tomorrow as well as the teachers of the system at various levels. Definitely it is you who decides whether the system is to flourish or to doom. As young Homoeopaths you have a still serious role to act as the system is on the verge of establishing its scientificity to the satisfaction of the so called learned men. Here please understand that Homoeopathy never lack the scientific approach but only the ‘learned men’ always failed to understand it or better said they pretend that they never understood it! But you can change that too with your more refined knowledge and yet more powerful tools available.

To make that decision of yours more worthwhile you have to digest some aspects that are peculiar to the medical profession.

With some luck anybody with basic qualifications can get past the barrier set for medical study that is the medical entrance examination. And except for a very few, all those who get into the medical colleges come out of it with a degree attached to their names and labeled as doctors. But the cardinal question is how many of them are re-formed into real medical men? Because prescribing or dispensing medicines is not much a Herculean task. Many of the less knowledged people are doing the same even without a four-letter tail attached to their name, under various systems of medicine. What makes the so called physicians different from them is not that four letter tail. But the rigorous and intense tests of humanity and kindness they get passed.

We should think aloud about all these now, because of the precarious situation of medical profession. You should well remember that the doctor is always in a position between man and God. Even if he is not the omnipotent it is he who delivers the life, health, disease and some times the death offered from above. The dignity and respect we get from all over underlines this point. But, to the greater misfortune of all of us the dignity of medical men is at the lowest now.  There are increasing number of reports of violence on doctors and hospitals. Equally increasing are the reports of indecent, unethical, indiscriminate practices from medical men. In short the interpersonal relationship between the man and the god-similar is deteriorating fast. There may be some aspect to the problem, which is out of our hands. But most of them can be solved easily and simply by some acts from our part.

There is a separate topic in the medical system called medical ethics that deals with the conduct and behaviour of the physician during his practice or rather his life time as a physician. Medical ethics is not a subject to be taught at the end of the curriculum. It s not at all a subject to be taught! Rather it should be a process in the minds of every medical man. A continued process that starts from the very first day of them stepping in to a medical college and ends only with their last breath.

To begin with one should have a clear perspective about why he/she is here. Because majority of the problems start with the fact that majority of the medicos are in the medical colleges for reasons other than to be true physicians. One can’t blame them either. They may be having a hundred reasons ranging from unfulfilled wishes from their parents’ to today’s ever-spiteful competition for a secure job. The result is that those with true inclination towards medical field and a matching mentality to serve the suffering humanity is getting a rare commodity. Not only the people but the medical field also bear the grudge of this often paradoxical situation as evident from the scenario around us.

Let’s emphatically remind ourselves that it is always the patients’ benefit for which we have to become physicians. Absolutely nothing holds water in against that! And despite the popular belief, in the medical field the path laid before you is full of thorns. From today onwards you are about to be disturbed from your normal life in countless number of times and ways! So prepare yourself for the inevitable. On the other hand you have the benefits too. Being part of the noblest profession available and countless privileges attached with it are only one portion of it. The real reward comes from the feeling of inner well being and intense satisfaction one gets after curing a sick person, after bringing one back from a suffering death or even accompanying one with soothing words and medication in to a pleasant journey from this world. Ironically these are available to only those medical men who are completely devoted to this profession. You have to take in this notion seriously from now itself even though your days of medical practice are a few years away.

How can you cultivate such an attitude? Start from the very first day itself. Understand that all the Anatomy, Physiology etc. you learn give you a very good insight about what is the man, in material! But always remember that there is a person behind every individual who is more than the Anatomy or Physiology. He/she is just like you and me, with a mind, feelings and emotions. Who suffers and enjoys. All the other details you learn are only to understand this ‘person’ in a better way. Because it is this ‘person’ you have to encounter in your career as a doctor, not the Anatomy or Pathology. Whoever brings in this solid fact in to their brain wins the first battle. To this you can add all the medical knowledge you acquire in your lifetime. Try to connect every bit of knowledge you receive with this ‘person’ so that it gets completed more and more in your imagination. Because no body has succeeded hundred percent in understanding this phenomena we called man. Next try to be open minded and challenging. At the same time keep your arrogance in the back stage so that by the time you are ready to get out of the colleges you will be having an aptitude towards the medical field.

Anybody knows that we can’t do a thing successfully if there is no interest in it. So as you have already entered yourselves in to this non-return path, you should as well acquire an interest in it too. For this focus on all the positive points of this profession and all the glory you can achieve by becoming a successful physician. And never approach this as a secure and successful career for this is only a service not a career! The remuneration part comes only the second. If you cannot cultivate a deep interest in medical filed it is highly recommended that you quit this programme altogether so that vast amounts of effort, money and time are not wasted of you and of the establishment!

If you did have the interest the next essential quality is the determination, to fight till the very end and to succeed. With this if you put in hard work the story will never be the same! Hard work means the real thing. Put every thing else aside and concentrate on your mission. From now onwards entertainment for you means getting more knowledge, more courage and more power in this battle against the evil that is unending.

Finally a word to all the professional organisations in Homoeopathy also. Let our mission be the one to enhance the progress of the system. Let it also help these young brigade to see clearly what is Homoeopathy and use it for the greater benefit of all. It is a happy thing to know that Kerala Homoeopathic Medical Graduate Association (KHMGA) is showing an appreciable model in this regard. KHMGA is doing a number of things ranging from keeping in constant touch with the student community, distributing prizes for excellence among them, constantly bringing updated knowledge through various seminars and symposiums and inviting and making them participate in various activities of the association.

This system is having powers still untapped. Pearls yet to be discovered. And a promise greater than everything offered so far to the suffering humanity. Lets work together to a brighter future for the medical fraternity and the system of Homoeopathy.
